Introduction Currently the ED does not stock a palatable liquid preparation of prednisone or prednisolone. The most palatable medication we stock is Prelone prednisolone syrup, 15 mg ml ; which is often rejected by children, and has a high incidence of vomiting after it is given. This is a problem in other EDs as well1. A better tasting version of prednisolone, brand name Orapred prednisolone sodium phosphate oral solution ; has recently appeared on the market and is widely available at area pharmacies. One solution we explored was to get Orapred added to the ED formulary, but the cost would be prohibitive. The alternative that we recommend is the use of oral decadron in the ED, with a follow-up prescription for Orapred for further outpatient doses. The use of oral decadron is well established in croup2, 3, and decadron use, both oral and IM has been studied in asthma as well1, 4. Formulation There are several oral decadron preparations on the market, but they are often low concentration and not any more palatable than the prednisone prednisolone preparations. However, the IV preparation of decadron is quite concentrated 4 mg ml or 10 mg ml ; and has been used orally1, 5. It is safe, and has good bioavailability6. Dosing Ddcadron is approximately 7-10 times as potent as prednisolone7, 8. Therefore, in order to achieve a similar effect to the traditional 2 mg kg dose of Prelone, a decadron dose of 0.2 to 0.3 mg kg should be used. Frequency Dscadron has a significantly longer half life than prednisolone; however this difference is not as pronounced when the IV preparation is used6. One study of decadron use in RAD compared two doses of decadron 0.6 mg kg ; on two consecutive days to the traditional 5 day Prelone regimen and found no difference1. Since we will not be dispensing the second dose of decadron from the ED, and our first dose 0.3 mg kg ; is calculated to be similar to 2 mg kg of Prelone, we recommend simply considering the initial dose of decadron as a substitute for the initial dose of prednisolone. Summary of Recommendations 1. In patients being treated for RAD exacerbations who are unable to swallow pills, an initial dose of 0.3 mg kg of decadron may be administered orally instead of 2 mg kg of Prelone. 2. This dose should be given using the IV solution of decadron administered orally. 3. The patient should then be prescribed the remainder of a course of outpatient oral steroids in the usual manner the dose of decadron in the ED can be considered equivalent to a 2 mg kg dose of prednisolone and rhinocort. 7. Systemic corticosteroid therapy should be reduced or eliminated gradually before reduction of RESPIHALER dosage is begun.
